Monogenic Diabetes
Gene: SH2B1
PMID 42764254 reports 5 individuals from 5 Chinese families with heterozygous variants (2 truncating de novo, 2 missense de novo, 1 maternally inherited missense). Phenotypes include developmental delay (5/5), speech delay (5/5), intellectual disability (5/5), ADHD/hyperactivity (5/5), ASD (3/5), and obesity/hyperphagia (1/5). The maternally inherited variant presented with obesity in the teenage child and mother. In HEK293T, truncating variants reduced protein expression and mislocalised protein, whereas the missense variants retained protein expression but had increased EGF-induced ERK phosphorylation.
PMID 23160192 reports 5 individuals from 5 mixed European families with heterozygous LOF variants (1 truncating, 3 missense). Phenotypes include early onset obesity, hyperphagia, insulin resistance, delayed speech and language development, social isolation, and aggressive behaviour. All variants were inherited from parents with phenotypic features. Functional assays showed all 4 variants significantly impaired the ability of SH2B1β to enhance neuronal differentiation compared with WT.Created: 15 Oct 2026, 12:59 p.m. | Last Modified: 15 Oct 2026, 12:59 p.m.

PMID 31439647 reports 16 unrelated individuals with severe early‑onset obesity, hyperphagia, insulin resistance and neurobehavioral abnormalities (learning difficulties, dyspraxia, ASD, ADHD, aggression/emotional lability, anxiety). They identified 15 rare missense variants in SH2B1 (12 heterozygous, 3 homozygous), but no segregation data or parental phenotype information is available. Seven of 15 variants were shown to impair the ability of SH2B1b to stimulate NGF-induced neurite outgrowth. Mouse models null for Sh2b1 exhibited obesity, impaired glucose homeostasis, and often, aggressive behaviour.
PMID 37329217 describes 4 individuals with severe early‑onset obesity, hyperphagia, insulin resistance and hepatosplenomegaly. They reported 5 missense variants (3 heterozygous, 1 compound heterozygous) but lacked segregation data and functional validation, and the variants were classified as VUS.
PMID 29216354 reports a single heterozygous missense variant whose allele frequency (0.47% in Africans) exceeds the dominant disease threshold and for which parental status is unknown.
